please help  me fixing this issue, were am I missing?

balance is 13 USD in Create proxies.

Also Vultr is alright as well.

the error is: Request failed: Server add failed: You have reached the maximum number of active and recently destroyed subscriptions for this account. To deploy additional services, please request a limit increase.

Hello Monty.

This error has nothing to do with CreateProxy. This error come back from provider as provider may limit number of instances you can create in specific timeframe. So the error is telling you that you are over this limits and you should request them for limit increase ot you will need to create another account for example.
